hey - not sure if this is the right channel to ask this, but i'm just looking for general information on how users of Medplum generate their drug formulary (i.e. MedicatonKnowledge FHIR resources) - particularly for customers that don't use major commercial drug databases (i.e. people that try to use RxNorm / NDC instead of FDB / Medispan). i read the relevant documentation that i could find ( [https://www.medplum.com/docs/medications/formulary](/content/docs/medications/formulary ""/index.html)) but would also be happy to be pointed to other docs to review.

more or less, i'm just curious - but also wondering what my org can do to make this easier since we are building an open drug data aggregator that can also transform open drug data into different views / flatfiles to possibly automate some of this work. thanks in advance for any insights!

I'm currently building a prototype with a Nuxt 3 frontend and a supabase database but very interested in transitioning my backend to MedPlum and maybe making the prototype an actual product. ChatGPT has been my software dev buddy so we'll see if I can figure out how to make it happen.

Hi folks, I'm exploring an idea in the health tech space and came upon Medplum as a promising option with its Typescript SDK for CRUD operation on FHIR resources. The documentation so far has been helpful; I had a few questions (a bit engineering focused) if someone has time to help out: 1. My goal is to store the FHIR data in a Postgres database I own via Supabase. I was looking into the self hosted option but it seems like there is a reliance on either running Postgres locally or using an AWS offering. What is the recommended path here? 2. When creating the MedplumClient, is it possible to connect directly to a Postgres server? My read of this is a no since it needs to connect to a FHIR server 3. I was looking at the self hosting option, but it seems like overkill since I don't really need to self host the whole Medplum application. I just need the FHIR api endpoint; is there a lightweight way of doing this with just a link to a Postgres db with creds? 4. Where does the parsing from a raw Postgres query response happen in the code? I've been searching the medplum repo but haven't seen it yet

@everyone I am having trouble running medplum standalone on my linux laptop. I have installed postgres, redis and node as described here: [https://www.medplum.com/docs/self-hosting/install-on-ubuntu](/content/docs/self-hosting/install-on-ubuntu ""/index.html) I think that following those instructions means that I can't use docker to start it up (or at least, running the docker command fails) but when I try to run the tests it also fails partway through. It seems that there are no tables or data in the DB. Any idea what I am doing wrong?

A Registry in medical data systems is a specialized highly focused repo of medical information about specific patients. For example, a cancer registry will have unique diagnosis metadata on thousands of patients with a specific cancer. Registries in medicine started long before registries in computer science, so it's important not to confuse the two as equal. My grandfather ran a tumor registry in his file cabinet back in the 1940s. Think of a medical registry as a very specific EHR these days. Sometimes I get so frustrated with mainstream EHRs in hospitals and clinics I want to quit trying to improve the ecosystem. It's just madness until you realize it's a matter of Vendor Lock In Syndrome (VLIS) which sadly has no cure. This paper https://pmc.ncbi.nlm.nih.gov/articles/PMC10853080/ gives hope. Registries and FHIR integration are living proof that it's time to recognize it is not worth the effort to bring big EHRs to the table and expect progress; the cure for VLIS is to simply start over and innovate. Big EHRs are literally on life support without hope of recovery. It's time to pull the plug.

Hi everyone, I'am new to contributing to Medplum and i'm running into a build issue after forking the latest main branch

when i run *npm run build *, it fails in several packages (like @medplum/agent and @medplum/server) with the typescript error TS2322: Type 'number' is not assignable to type 'Timeout' on lines that use setInterval or setTimeout

I've done some trouble shooting already;

  1. I was inititally on a unstable nodejs version(v23.12.0),but i've switched to a suported version (v22.12.0) and did a clean *npm install *

  2. I was able to fix the build errors locally by changing the type declaratino from NodeJS.Timeout to ReturnType in the affected files (e.g: packages/agent/src/app.ts,packages/agent/src/app.ts,

My question is : Is this a known issue for new contributors setting up the project locally? And would a pull request with these ReturnType fixes be a welcome contribution to make the setup process smoother for others?

I just came across AWS Health Connect https://docs.aws.amazon.com/connecthealth/latest/userguide/what-is-service.html it looks like it has a FHIR integration. > EHR integration > Amazon Connect Health connects to your EHR system through FHIR R4 APIs. This integration enables bidirectional data exchange for patient verification, appointment management, and clinical data retrieval. Amazon Connect Health currently supports integration with Epic EHR systems. I was wondering if anyone has tried tying that into their Medplum instance ?

His arguments that hacking is a bigger issue now aren't particularly compelling, like claiming that anyone with an AI agent can hack an open source codebase. Development has accelerated equally fast on both sides of the security/hacker equation.

However, before AI it was much harder to self-host, so the value of the cloud version of cal.com was much higher. Now it's trivially simple to self-host, especially in the past few months as AI gets better using CLI tools like AWS CLI. So I'm guessing the ratio of cloud vs. self-hosted installs is looking alarmingly different to him and his investors than it was even a year ago, which is impacting growth.

What do you think?

this feels less like a security decision and more like a revenue protection strategy. I agree with you, it does dramatically reduce the friction of self-hosting. If anything, open source + AI should make systems more secure over time, not less.
